- The CRC form can be downloaded by clicking the above link or is available at Nancy Moore's residence, 2460 Leclair Drive, Coquitlam. There is a box outside the front door with blank ones in it.
- The Letter of request must be brought along with the CRC form to the RCMP. You will need to initial the 4 boxes in sections 1 through 4.
After RCMP processing, bring the forms to Nancy Moore, Risk management coordinator (adress above).

Note: Form A is required for the adult women's and men's teams as well as a few under-age coaches, who will be advised separately to acquire this form.
